Faith Formation

No matter how old or young, everyone is involved together in a "journey of a lifetime" in exploring our Catholic faith.  As seen below, Saint Sebastian offers many services and programs involving religious education and faith formation.  If you are interested in obtaining more information, becoming involved, or volunteering in one or more of these programs, please click here to fill out an interactive form.  You may also click here (Adobe Reader required) to print and fill out a form.  If you have any questions, please contact the Parish Office.
